Role: RN Case Manager — Hospice
Location/Type: Victoria, TX • Field-based • Full-time
Pay: $75,000–$85,000 + on-call pay + mileage
Schedule: Monday–Friday, 8:30 AM–5:30 PM
What you'll do

Manage a hospice caseload across Victoria-area communities.

Complete nursing assessments in homes and care settings.

Build care plans with MD, MSW, chaplain, and CNA.

Teach families pain, symptom, and medication management.

Keep EMR charting accurate and on time.

Join a shared on-call rotation with fair coverage.
Must-haves
-
Active Texas RN license.

Able to work field-based across Victoria-area communities.

Comfortable teaching families during difficult moments.

Able to manage documentation in an EMR.

Ready for weekday and weekend on-call rotation.

Must live within reasonable distance of Victoria.
Nice to have

Hospice experience.

Home health experience.

SNF, LTC, ER, ICU, or step-down background.
