Role: RN Case Manager — Hospice
Location/Type: Victoria, TX • Field-based • Full-time
Pay: $75,000–$85,000 + on-call pay + mileage
Schedule: Monday–Friday, 8:30 AM–5:30 PM

What you'll do

  • Manage a hospice caseload across Victoria-area communities.
  • Complete nursing assessments in homes and care settings.
  • Build care plans with MD, MSW, chaplain, and CNA.
  • Teach families pain, symptom, and medication management.
  • Keep EMR charting accurate and on time.
  • Join a shared on-call rotation with fair coverage.

Must-haves

  • Active Texas RN license.
  • Able to work field-based across Victoria-area communities.
  • Comfortable teaching families during difficult moments.
  • Able to manage documentation in an EMR.
  • Ready for weekday and weekend on-call rotation.
  • Must live within reasonable distance of Victoria.

Nice to have

  • Hospice experience.
  • Home health experience.
  • SNF, LTC, ER, ICU, or step-down background.
  • Bilingual skills.

Perks & pay

  • Pay: $75,000–$85,000 based on experience.
