What does a Transitions at Home RN Case Manager do? Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, careplan review, incident investigation, and Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
