Responsibilities

• Conduct intake assessments with patients and families.

• Coordinate patient care with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
• Ensure patients have access to necessary healthcare services and resources.
• Educate patients and their families about managing illnesses and treatment plans.
• Advocate for patients' needs with insurance providers and healthcare facilities.

Requirements

• Certification: Licensed Registered Nurse with no restrictions.
• Education: Bachelor's degree in nursing required; advanced degree preferred.
• Experience: At least 1 year as an RN Case Manager or similar role in hospice care.
• Skills:

  • Ability to work independently and as part of a team.
