Job Title: RN Case Manager
Location: Los Angeles, CA 90034
Duration: 16+ Weeks | Possible Extension
Shift: Days | 8:00 AM – 4:30 PM | 8×5 | 40 Hours/Week
Pay: Local: $90/hr on W2
Travel: $3500/Weekly

Job Summary:

• Coordinate patient care and support services.
• Assess patient needs and develop individualized care plans.
• Coordinate with physicians, healthcare teams, patients, and families.
• Monitor patient progress and facilitate referrals and resources.
• Maintain accurate documentation and follow healthcare standards.

Requirements:

• 1+ year of recent Case Management experience within the last 3 years
• Previous experience in Utilization Review & Discharge Planning.
• BLS
