The Sisters of Mercy of the Americas is seeking an experienced RN Case Manager to support the health, wellness, and independence of Sisters living in our residence and surrounding community.
This role combines nursing, case management, advocacy, education, and care coordination. The RN Case Manager works closely with individual Sisters, the Health and Wellness team, medical providers, and community resources to ensure each Sister receives compassionate, coordinated care that supports her individual needs and goals.

Conduct scheduled wellness and health assessments and identify changing health needs.
Provide case management for chronic conditions, new diagnoses, and progressive illnesses.
Develop and implement individualized care and service plans and participate in discharge planning.
Coordinate care with medical providers, specialists, rehabilitation services, behavioral health providers, homecare agencies, and other community resources.
Accompany Sisters to medical appointments when needed and help them navigate the healthcare system.
Coordinate follow-up appointments, transportation, and communication between Sisters, the Health Office, and healthcare providers.
Provide hands-on nursing care, including vital signs, health assessments, wound care, injections, and first aid.
Respond calmly and effectively to health-related emergencies and crisis situations.
Provide education, counseling, and support on health and wellness topics in collaboration with the care team.
Help coordinate care for Sisters living in the outside community.
Build strong relationships with internal and external healthcare providers to promote continuity and quality of care.
Advocate for each Sister's health, wellness, autonomy, and individual needs.
Maintain accurate documentation and follow established safety and care procedures.
The ideal candidate is an experienced RN who brings strong clinical judgment, organization, compassion, and a genuine interest in supporting older adults and their overall well-being.
Required:
Current Registered Nurse license; BSN preferred.
At least 5 years of professional nursing experience.
Strong experience in geriatric care, long-term care, homecare, or a related healthcare setting.
Experience with case management and admission, transfer, and discharge planning.
Ability to work independently while also collaborating effectively as part of a multidisciplinary team.
Strong organizational skills and attention to detail.
Ability to respond calmly and effectively during emergencies and changing health situations.
Strong communication and interpersonal skills.
Commitment to a holistic approach to health and wellness.
Comfort working in a healthcare environment serving religious women.
