RN Case Manager

Location: Milford, MA

Rate: Up to $2,300 Gross Weekly + benefits available from day 1

Shift: Days, 5x8-hour shifts, 8:00 AM–4:30 PM; every third weekend required

Contract: 10 weeks

Hours: 40 hours/week

Requirements:

• 3–5 years of experience required
• BSN required
• Massachusetts RN license required
• Acute care experience required
• Case management/utilization review experience
• Care coordination and discharge planning experience
• Epic experience preferred
• Local and travel candidates accepted

Certifications:

• BLS required
• CCM preferred

Responsibilities:

• Provide case management services for adult and geriatric patients
• Coordinate patient care and discharge planning
• Complete patient needs assessments and coordinate DME orders
• Support care coordination across the acute care setting
• Apply CMS and HIPAA guidelines to patient care and documentation
• Collaborate with interdisciplinary teams to support safe and appropriate patient outcomes
