RN CASE MANAGER Multiple Locations- Newport Beach, San Marcos, West Hills, Culver City, CA
Overview
We are seeking skilled RN Case Managers to deliver comprehensive nursing care and case management services in a home health setting across multiple Southern California locations. This role offers meaningful hands-on work with strong job stability, solid compensation, and excellent benefits.
About the Role
As an RN Case Manager, you will serve as the primary clinical leader for assigned patients, conducting comprehensive assessments, developing individualized care plans, and delivering direct skilled nursing care while supervising support staff. This is a full-time, direct hire position with relocation assistance available.
Key Responsibilities
Clinical Assessment & Care Planning:
• Conduct comprehensive nursing assessments of patients' physical, emotional, psychosocial, and spiritual needs during admission, resumption of care, recertification, and discharge
• Develop, implement, revise, and evaluate individualized Plans of Care in collaboration with patients, families, physicians, and the interdisciplinary team (IDT)
• Perform accurate and timely OASIS assessments to ensure proper reimbursement, quality reporting, and optimal patient outcomes
Direct Patient Care:
• Deliver direct skilled nursing care, including wound care, medication administration and management, IV therapy, injections, symptom management, and patient/caregiver education
• Educate patients and their families/caregivers on disease management, medication adherence, safety measures, self-care techniques, and strategies to prevent re-hospitalizations and promote independence
• Monitor patient progress, promptly identify changes in condition, and adjust care plans accordingly to optimize outcomes
Clinical Leadership & Supervision:
• Supervise and direct LPNs/LVNs, home health aides, and other support staff to ensure coordinated, high-quality care delivery
• Participate in case conferences and interdisciplinary team meetings
• Manage a productive caseload with efficient visit scheduling within the assigned territory
Documentation & Compliance:
• Maintain accurate, timely clinical documentation in the Electronic Medical Record (EMR) system in compliance with agency policies, OASIS guidelines, and all regulatory/payer requirements
• Actively participate in quality improvement initiatives
Other Responsibilities:
• Respond to urgent patient needs and participate in on-call rotations as required
Requirements
Education & Licensure (Required):
• Graduate from an approved school of professional nursing
• Currently licensed to practice as a registered nurse in California
• One (1) year experience as a RN (preferred 1-2 years)
Certifications & Requirements:
• Valid California RN license (required)
• Current CPR certification (required; must be maintained during employment)
• Valid driver's license with insured automobile in good working order
• Ability to travel to all business locations

Compensation
• Base Salary: $114,340.66 - $128,652.29 annually
