About the Role

Our client is seeking an experienced Registered Nurse (RN) Field Case Manager to join their growing home health team serving Palm Beach and North Palm Beach County. This is a full-time, field-based position for an RN who enjoys working directly with patients while maintaining ownership of their care from admission through discharge.

The RN Field Case Manager will provide skilled nursing services in patients' homes while coordinating the overall plan of care. This position works closely with physicians, therapists, home health aides, patients, families, and the clinical team to ensure each patient receives safe, appropriate, and well-coordinated care.

The ideal candidate is an organized, compassionate RN who is comfortable working independently in the field and understands the clinical and documentation requirements of a Medicare-certified home health environment.

Responsibilities

• Provide skilled nursing care to patients in their homes throughout the assigned Palm Beach County territory.

• Manage an assigned caseload and oversee each patient's clinical care from admission through discharge.

• Complete comprehensive nursing assessments and identify changes in patient condition.

• Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.

• Complete OASIS assessments and maintain accurate, timely clinical documentation.

• Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.

• Provide skilled nursing services including medication management, wound care, disease management, patient education, and other treatments within the RN scope of practice.

• Monitor patient progress and communicate changes in condition to physicians and appropriate members of the care team.

• Coordinate services with Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aides, and other disciplines as needed.

• Educate patients and caregivers regarding medications, diagnoses, treatment plans, safety, and ongoing care needs.

• Identify potential complications, hospitalization risks, and changes in condition and intervene appropriately.

• Participate in case conferences and collaborate with clinical leadership regarding patient needs and progress.

• Ensure all documentation is completed accurately and within required agency and regulatory timeframes.

• Maintain compliance with Medicare Conditions of Participation, agency policies, and applicable state and federal regulations.

• Manage scheduling and travel efficiently throughout the assigned territory while maintaining appropriate patient communication and follow-up.

Qualifications

• Active and unrestricted Florida Registered Nurse (RN) license required.

• Previous nursing experience in Medicare-certified home health strongly preferred.

• Experience completing OASIS assessments preferred.

• Strong clinical assessment and critical-thinking skills.

• Ability to independently manage a patient caseload and prioritize visits based on clinical needs.

• Strong documentation, organization, and time-management skills.

• Comfortable working independently in a field-based environment.

• Excellent communication skills with patients, families, physicians, and interdisciplinary care teams.

• Valid driver's license, reliable transportation, and ability to travel throughout the assigned Palm Beach / North Palm Beach County territory.

• Current CPR/BLS certification preferred.

Compensation & Benefits

• $75,000 $85,000 base salary, depending on experience.
