Travel Registered Nurse (RN) – Hospice Case Manager

Job Summary
Join a compassionate hospice team dedicated to providing exceptional end-of-life care. As a Travel RN Hospice Case Manager, you will coordinate patient and family care, perform comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to maximize patient comfort and quality of life.
This role is ideal for an RN who is passionate about patient advocacy, education, care coordination, and delivering compassionate hospice services.
Key Responsibilities
Provide comprehensive physical, psychosocial, and spiritual patient assessments.
Develop, implement, and update individualized patient care plans.
Coordinate care with physicians, families, interdisciplinary teams (IDT), and partnering facilities.
Educate patients, caregivers, and families on disease management, medications, and hospice care.
Administer and monitor medications as prescribed.
Ensure accurate and timely clinical documentation.
Promote patient safety and infection prevention practices.
Supervise LPNs and Home Health Aides according to agency policies.
Participate in interdisciplinary team meetings and care planning.
Provide bereavement support following a patient's passing.
Maintain compliance with hospice regulations, Joint Commission standards, and organizational policies.
Participate in a 24/7 patient care rotation as assigned.
Maintain professional licensure, certifications, and continuing education requirements.
Minimum Qualifications
Associate's Degree in Nursing (or equivalent)
Active Florida Registered Nurse (RN) License
Valid Florida Driver's License
Current BLS Certification (American Heart Association or American Red Cross)
Reliable transportation with valid automobile insurance
Minimum of one (1) year of nursing experience or an equivalent combination of education and experience.
Preferred Qualifications
One (2) years of experience in an acute care or post-acute care setting
Hospice or palliative care experience
Experience caring for terminally ill patients
Typing proficiency of at least 25 WPM
