RN Case Manager – Pediatric Home Health (IHSS) | Hybrid

WSi is partnering with a pediatric home health organization expanding their IHSS program in Colorado and seeking an experienced Registered Nurse Case Manager! This hybrid role offers the opportunity to make a real impact supporting children and families through hands-on supervision, care coordination, and caregiver support in the home.

Position Highlights:

• Hybrid schedule: 4 days in the field, 1 day remote
• Oversee pediatric IHSS cases and provide in-home supervision
• Conduct skills validation and competency checks for caregivers
• Review care plans and ensure adherence to IHSS services
• Investigate and resolve client or caregiver concerns
• Collaborate with families and care teams
