Your day-to-day includes:Developing and executing individualized care plans tailored to each patient's medical, functional, and emotional needsLeading interdisciplinary collaboration with PTs, OTs, social workers, home health aides, and physiciansConducting in-home patient assessments and adjusting care plans as conditions evolveEducating patients and families on disease management, medications, and self-careCompleting accurate, timely EHR documentation in compliance with Medicare and state regulationsTracking KPIs including visit completion rates, clinical outcomes, and patient satisfaction scoresReducing hospital readmissions and improving functional independence through evidence-based careWhat You BringEducation: Associate's or Bachelor's degree in Nursing (BSN preferred)Licensure: Active RN license in Arizona (required)Experience: 2+ years of clinical nursing experience; 1+ year in home health preferredSkills: Strong assessment, care planning, time management, and communication skillsTech: Comfortable with EHR systems; home health software experience is a plusRegulatory Knowledge: Familiarity with Medicare/Medicaid home health standardsMindset: Patient advocate, team player, and adaptable in a fast-paced field environmentWork EnvironmentPrimarily in patients' homes throughout the Phoenix metro areaOccasional office visits for team meetings and trainingFast-paced, autonomous, and deeply rewardingHow We Measure Your SuccessYour impact is real and trackable:Visit completion rates & documentation timelinessPatient satisfaction scoresClinical outcomes & reduction in hospital readmissionsContribution to a cohesive, collaborative care teamPatient empowerment and improved quality of lifeOur Commitment to Inclusion & BelongingAmerican Premier is dedicated to fostering a diverse and inclusive workforce.
