Responsibilities

• Coordinate patient scheduling, messages, records, and follow‑up activities to support efficient care delivery.

• Communicate with patients, providers, and staff to ensure accurate information exchange and smooth transitions.

• Maintain documentation, assist with regulatory compliance needs, and support daily operational workflows.

Minimum Requirements

\n

• \n
• 1 year as RN Case Manager (Acute/Hospital) – current/recent
• \n
• Active RN Oregon & Washington/Compact license
• \n
• BLS - AHA
• \n
