HIRING! For an RN Case Manager in the home health industry. You will coordinate and advocate for patients, ensuring comprehensive and individualized care is provided within their homes. You will develop and implement care plans, collaborate with healthcare providers, and educate patients and families on their conditions and treatment options.

Responsibilities:
  • Patient Assessment and Care Planning:
    • Conducts initial and ongoing assessments to determine the patient's needs and develop a personalized plan of care. 
    • Collaborates with the patient, family, and other healthcare providers to establish goals and priorities. 
    • Ensures the care plan aligns with the patient's preferences and addresses their physical, emotional, and social needs. 
  • Care Coordination and Supervision:
    • Coordinates the delivery of care, including delegating tasks to other healthcare professionals (e.g., physical therapists, home health aides). 
    • Monitors the patient's progress and makes adjustments to the plan of care as needed. 
    • Supervises and supports the home health team, ensuring quality and adherence to protocols. 
 
  • Communication and Documentation:
    • Communicates regularly with patients, families, physicians, and other healthcare providers to facilitate care coordination. 
    • Maintains accurate and complete documentation of patient assessments, care plans, and progress. 
    • Participates in team meetings and case conferences to share information and collaborate on care. 




 
  • Patient and Family Education:
    • Provides education to patients and families on their conditions, treatments, and home care procedures. 
    • Addresses questions and concerns, offering support and guidance throughout the care process.

  • Advocacy and Resource Management:
    • Serves as a patient advocate, ensuring their needs and preferences are considered. 
    • Identifies and facilitates access to community resources and support services. 
    • May be involved in discharge planning and transitioning patients back to their homes or other settings.

Skills and Qualifications:
  • Registered Nurse License (RN): Current and valid RN license in the state of CA.
  • Clinical Skills: Proficiency in conducting assessments, implementing care plans, and monitoring patient status. 
  • Communication Skills: Excellent interpersonal and communication skills to effectively interact with patients, families, and other healthcare professionals. 
  • Organizational Skills: Ability to manage a caseload, coordinate care, and prioritize tasks. 
  • Problem-Solving and Decision-Making: Ability to analyze situations, identify problems, and make sound decisions in a timely manner. 
  • Knowledge of Home Health Regulations: Familiarity with relevant regulations and standards related to home health care. 
  • Experience in Home Health: Preferred, but not always required, with experience in home health settings. 
  • Education: Associate's or Bachelor's degree in Nursing (BSN) preferred. 
  • Technology: WellSky, Kinnser, Oasis, MS Office Suite

OASIS Admission 2.50 hours at $60.00 HR
OASIS Resumption of Care 2.50 hours at $54.00 HR
OASIS Recertification 1.50 hours at $80.00 HR
OASIS Discharge 1.0 hour at $100.00 HR
OASIS Non-billable D/C 0.50 hours at $80.00 HR
OASIS Transfer 0.25 hours at $40.00 HR
Skilled Nursing Evaluation 1.50 hours at $66.67 HR
Skilled Nursing Evaluation + Admission 1.50 hours at $73.33 HR
Skilled Nursing Assessment 2.0 hours at $50.00 HR
Follow Up Visit 1.0 hour at $80.00 HR
Visit + Discharge Summary 1.0 hour at $80.00 HR
Meeting/Orientation/In-Service/PTO $50.00 HR
Drive time $17.25 HR
