You will develop and implement care plans, collaborate with healthcare providers, and educate patients and families on their conditions and treatment options.

Responsibilities:- Patient Assessment and Care Planning:
- Conducts initial and ongoing assessments to determine the patient's needs and develop a personalized plan of care.
- Collaborates with the patient, family, and other healthcare providers to establish goals and priorities.
- Ensures the care plan aligns with the patient's preferences and addresses their physical, emotional, and social needs.
- Care Coordination and Supervision:
- Coordinates the delivery of care, including delegating tasks to other healthcare professionals (e.g., physical therapists, home health aides).
- Monitors the patient's progress and makes adjustments to the plan of care as needed.
- Supervises and supports the home health team, ensuring quality and adherence to protocols.
- Communication and Documentation:
- Communicates regularly with patients, families, physicians, and other healthcare providers to facilitate care coordination.
- Maintains accurate and complete documentation of patient assessments, care plans, and progress.
- Participates in team meetings and case conferences to share information and collaborate on care.
- Patient and Family Education:
- Provides education to patients and families on their conditions, treatments, and home care procedures.
- Addresses questions and concerns, offering support and guidance throughout the care process.

- Advocacy and Resource Management:
- Serves as a patient advocate, ensuring their needs and preferences are considered.
- Identifies and facilitates access to community resources and support services.
- May be involved in discharge planning and transitioning patients back to their homes or other settings.

Skills and Qualifications:- Registered Nurse License (RN): Current and valid RN license in the state of CA.
- Clinical Skills: Proficiency in conducting assessments, implementing care plans, and monitoring patient status.
- Communication Skills: Excellent interpersonal and communication skills to effectively interact with patients, families, and other healthcare professionals.
- Organizational Skills: Ability to manage a caseload, coordinate care, and prioritize tasks.
- Problem-Solving and Decision-Making: Ability to analyze situations, identify problems, and make sound decisions in a timely manner.
- Knowledge of Home Health Regulations: Familiarity with relevant regulations and standards related to home health care.
- Experience in Home Health: Preferred, but not always required, with experience in home health settings.
- Education: Associate's or Bachelor's degree in Nursing (BSN) preferred.
