Requirements

• Minimum 3 years clinical experience in this specialty required
• 3–5 years experience in a similar hospital setting (academic / large Trauma hospital required)
• Prior travel required: 2–3 travel assignments
• Epic experience required
• Florida License required at time of submission

Special Skills / Experience

• Minimum 3 years Acute Care Case Management experience
• Knowledge of:
  • Discharge planning *******
  • Payer requirements
  • Regulatory standards
  • Community resource navigation
  • Transitions of care processes
• Ability to:
  • Perform comprehensive patient assessments
  • Collaborate and communicate with physicians, community partners, and patients/families

Job Description

Responsible for assessment, planning, and implementation of the plan of care for assigned patients.

Works collaboratively with the interdisciplinary team (IDT) while focusing on patient progression throughout hospitalization, appropriate resource utilization, and ensuring a safe, effective transition from the hospital.
