Job Title: RN Case Manager

- BLS and Active CA license
- RN Case Manager
- Years of experience 2 years acute care case management/discharge planning
- Weekend REQ: Every other weekend. Must be available for holidays
- CA state license: Yes
- Type of Facility: Acute Care Facility
