Job Summary
We are seeking an experienced Travel RN Case Manager to join a leading acute care hospital in Fairfield, California. The ideal candidate will be responsible for coordinating patient care across the continuum, ensuring appropriate utilization of healthcare resources, facilitating safe discharge planning, and collaborating with multidisciplinary teams to achieve optimal patient outcomes.
This position requires strong clinical judgment, exceptional communication skills, and expertise in acute care case management.
 
Responsibilities:
  • Perform comprehensive patient assessments and develop individualized case management plans.
  • Coordinate care across the continuum, including discharge planning and transitions of care.
  • Conduct utilization review and ensure medical necessity using MCG guidelines.
  • Collaborate with physicians, nurses, social workers, and interdisciplinary teams to optimize patient outcomes.
  • Coordinate referrals to post-acute care, rehabilitation, hospice, home health, skilled nursing facilities, and community resources.
  • Ensure compliance with Medicare, Medi-Cal, Joint Commission, and regulatory requirements.
  • Document case management activities accurately within the electronic medical record.
  • Monitor patient progress and revise care plans as appropriate.
  • Educate patients and families regarding discharge plans, available resources, and ongoing care needs.
  • Promote quality outcomes, patient safety, and cost-effective care throughout hospitalization.
 
Minimum Qualifications:
  • Current California Registered Nurse (RN) License (Required)
  • Minimum 2 years of recent acute care nursing experience
  • Recent Hospital Case Management experience strongly preferred
  • Graduate of an accredited Nursing program
  • BSN preferred (MSN is a plus)
  • CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification preferred
