We are seeking an experienced RN Case Manager to support inpatient case management services at Fairfield, CA. The RN Case Manager will assess, plan, coordinate, implement, monitor, and evaluate individualized patient care plans across the continuum of care.
The Case Manager will collaborate closely with physicians, nurses, Social Services, patients, families, and other healthcare professionals to coordinate appropriate care, facilitate safe discharge planning, and optimize patient outcomes.
Key Responsibilities
Perform comprehensive patient assessments and reassessments throughout the hospital stay.
Develop and coordinate individualized plans of care and discharge plans.
Manage assigned patients across acute care units, which may include Med-Surg, Med-Tele, Surgical, ICU, and potentially ED.
Collaborate with physicians, nursing staff, Social Services, and other interdisciplinary team members.
Evaluate medical necessity, utilization, and appropriate levels of care.
Coordinate transitions of care across acute care, home health, subacute, long-term care, hospice, rehabilitation, and community resources.
Apply critical thinking and sound clinical judgment to resolve patient care and discharge barriers.
Utilize clinical knowledge, communication, organization, and time-management skills to prioritize daily assignments.
Support quality outcomes while considering healthcare resources, financial considerations, and health-plan requirements.
Maintain compliance with applicable healthcare regulations, organizational standards, and patient-safety requirements.
Demonstrate professionalism, teamwork, communication, collaboration, and customer-service skills.
Required Qualifications
Current California RN License.
BSN required.
Minimum 2 years of acute clinical nursing experience.
Minimum 2 years of Case Management experience.
Graduate of an accredited nursing school or college.
Strong inpatient/acute-care case management experience.
Ability to work independently with minimal supervision.
Strong assessment, critical-thinking, communication, organizational, and problem-solving skills.
ED experience is highly preferred.
Travel nursing experience and/or California healthcare experience strongly preferred.
