Vitality at Home is seeking a Registered Nurse (RN) Case Manager in Virginia, MN. The RN Case Manager plays a vital role in ensuring the health, safety, and well-being of home care patients through comprehensive assessments, individualized care planning, ongoing monitoring, and staff supervision. This role supports each patient in achieving their highest level of independence while maintaining compliance with state and federal regulations.

Required Skills & Abilities
Strong knowledge of adult clinical care and home care regulations
Proficiency with Windows-based systems and healthcare software
Excellent planning, time management, and ability to work independently
Strong written and verbal communication skills (English proficiency required)
Professional demeanor with integrity, flexibility, and confidentiality

Ability to manage multiple tasks, prioritize effectively, and problem-solve
Strong attention to detail and ability to recall information accurately

Education & Experience
Current Registered Nurse (RN) license in the State of Minnesota
Two years of experience in home health, med-surg, or acute care preferred
