Job Description:

Position Summary:The Registered Nurse Non-Certified Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. **Facility reserves the right to cancel three (3) shifts per 13-week contract in the event of low census or other unforeseen circumstances. Traveler must live at least 90 miles away from the facility to be considered- Do not submit a candidate if they do not meet this requirement.
