Description: Our client is currently seeking a RN - CASE MGT - Lafayette, LA

Registered Nurse – Care Management
Shift: 8:00 AM – 4:30 PM

Job Summary: This position serves as a leader in the delivery of patient care, utilizing the Nursing Process within the framework of the Nurse Practice Act, ANA Code for Nurses, and Scope & Standards of Practice. The Registered Nurse (RN) delegates, directs, and supports licensed and ancillary team members, assumes accountability for quality patient outcomes, and demonstrates cultural sensitivity in all interactions. The RN actively contributes to unit and departmental goals and upholds the organization’s philosophy of nursing.

Job Duties:

• Manages key components of the care planning process as patients move through the healthcare system
• Applies care management policies, evidence‑based criteria, and levels of care to ensure appropriate utilization
• Coordinates and manages care for assigned patients based on individualized needs
• Develops, implements, and reinforces post‑discharge care plans
• Adapts communication and behavior to meet the needs of diverse patient populations, including privacy, consent, and explanation of services
• Performs additional related duties as assigned

Qualifications: 
Education Required: Registered Nurse Diploma
Preferred: Associate’s or Bachelor’s degree in Nursing
Experience Required: Minimum of 2 years of related experience

Certifications Required:
-Current RN License in the state of practice
-Current Basic Life Support (BLS) certification from the American Heart Association
Preferred:
-Certification in Case Management (CCM)
-Certification in Health Care Management (CPHM)
