Support providers by gathering, collecting and clarifying information from patient,
family members, or nursing facility staff and accurately communicating to the provider. Assesses
patient condition and follows provider approved protocols and make disposition or escalation
decisions efficiently and accurately.
SUPERVISOR: Practice Manager
DUTIES INCLUDE BUT NOT LIMITED TO:
· Conduct initial assessments of patients, either in person or over the phone, to determine the urgency and severity of their condition.
· Gather pertinent information regarding the patient's medical history, current symptoms, and any relevant medications.
· Utilize evidence-based guidelines and clinical judgment to prioritize patient needs and determine the appropriate course of action.
· Provide accurate and timely medical advice and guidance to patients, caregivers, and healthcare professionals.
· Collaborate with healthcare teams, including doctors, nurses, and other medical professionals, to ensure coordinated and effective patient care.
· Document patient interactions, assessments, and recommendations in electronic medical records systems.
· Monitor and reassess patients as necessary, ensuring appropriate follow-up care and referrals.
· Stay up to date with current medical practices, guidelines, and regulations to provide the highest standard of care.
· Maintain patient confidentiality and adhere to ethical and legal standards in healthcare.
· Performs other related job functions as assigned
EDUCATION:
• Graduate of an accredited Registered Nurse School of Nursing required
EXPERIENCE:
· A minimum of one-year previous nursing experience in a clinical or hospital setting required
· Triage or emergency department experience preferred
CERTIFICATION/LICENSE:
· Current and unrestricted Register Nurse license in the respective jurisdiction per state guidelines required. Nurses are governed by their state board of nursing and work under their own individual license. LPNs may be qualified to perform triage based on state law.
· Certification in Basic Life Support (BLS) is preferred.
