Join the MercyOne Family! We are looking to hire an RN- Clinical Coordinator for our MercyOne Iowa Heart Advanced Heart Failure Clinic.
As a RN Clinical Coordinator at MercyOne, you will Coordinates comprehensive, specialized care for heart failure patients being evaluated, treated, and managed as an inpatient at MercyOne Des Moines Medical Center.. Collaborates with multitude of services including but not limited to the MercyOne Des Moines hospitalist program, critical care program, and various disciplines within the cardiovascular service line to, educate and coordinate care along the continuum. The primary goal is to prevent heart failure readmissions by participating in multidisciplinary collaboration including but not limited to the hospitalists, cardiologists, pharmacists, social workers, and the paramedicine program, to collaboratively manage the patient transition from inpatient to outpatient.
