Serve as an initial introduction to hospice and palliative care for patients and families
Assess patients for hospice eligibility and help patients and families understand their care options
Conduct information and sign-up visits and document appropriately in the EMR
Conduct hospice admissions on an as-needed basis throughout the Good Samaritan service area
Coordinate admissions for patients transitioning from the hospital to home or a facility, including DME, medications, transportation, discharge orders, and caregiver needs
Assess patients for General Inpatient (GIP) eligibility and complete GIP admission assessments as requested
Serve as the RN Case Manager for GIP patients and coordinate their plan of care with the interdisciplinary team
Educate hospitalists, discharge planners, nurses, and other healthcare professionals on hospice eligibility, the referral process, symptom management, and reimbursement
Develop and maintain strong relationships with hospital staff, discharge planners, physicians, and other healthcare and community partners
Serve as a clinical and educational resource regarding hospice appropriateness and the referral process
Promote Good Samaritan programs and services through professional development, community education, health fairs, and other educational initiatives
Work collaboratively with hospital teams to support hospice and palliative care protocols
Communicate patient information, goals, treatment plans, and referral needs effectively with the interdisciplinary team
Maintain timely and accurate documentation, tracking, and reporting
Communicate with the Good Samaritan Medical Director and/or Nurse Practitioner as needed
Participate in admission nurse meetings, team meetings, continuing education, and professional development
What we are looking for:
Graduate of an accredited nursing program with an active Virginia RN license
Three years of nursing experience; community health, hospice, and/or ICU experience preferred
Strong clinical assessment and critical-thinking skills
Excellent verbal and written communication skills
Ability to educate physicians, nurses, discharge planners, hospital staff, patients, and families about hospice care
Working knowledge of EMR systems and technology
Interest in healthcare, community resources, sales, marketing, and public relations concepts
Ability to work flexible hours, including occasional evenings and weekends
Reliable transportation, valid driver's license, and driving record that meet Good Samaritan requirements
Willingness to develop General Inpatient (GIP) eligibility and admission proficiencies within 90 days of hire
Schedule & Benefits
Schedule:M-F 8:30a-5p with occasional evenings and weekends. This position requires daily travel throughout the Good Samaritan Hospice service area and primarily works in hospitals and long-term care facilities.
