RN Clinical Liaison – Hospital to Home Health

Home Health Advantage, Inc., an established Medicare-certified home health agency, is seeking an energetic and highly personable Registered Nurse (RN) to join our team as an RN Clinical Liaison.

This is a unique role for an RN who enjoys working with people, building professional relationships, and helping patients transition safely from the hospital to home. This is not a traditional bedside or field nursing position.

Responsibilities

• Meet with prospective patients and families in the hospital prior to discharge.
• Explain home health services and help patients understand what to expect after returning home.
• Coordinate referrals and transitions of care with hospital case managers, discharge planners, physicians, and our internal clinical team.
• Maintain excellent relationships with hospital staff and referral partners.
• Communicate referral information accurately and promptly to our intake and clinical teams.
• Follow patients through the discharge and admission process to help ensure a smooth transition home.
• Utilize electronic medical records, referral platforms, email, and other technology throughout the workday.
• Represent Home Health Advantage professionally within the hospital environment.

Qualifications

• Current Illinois RN license.
• Exceptional interpersonal and communication skills.
• Excellent computer and technology skills.
• Professional, energetic, confident, and approachable personality.
• Strong organizational skills and attention to detail.
• Ability to communicate comfortably with patients, families, physicians, case managers, and other healthcare professionals.
• Home health, hospital, case management, intake, or care coordination experience is preferred but not required.
• Familiarity with Medicare home health services is a plus.
