RN - Community Hospice House Full time

Full time 36 or 40 hours per week. Schedule will either be 7:00pm- 7:00am or 11:00pm- 7:00am including every other weekend rotation & holiday coverage.

About the Job

The Community Hospice House recently completed a transformative expansion and renovation project to meet the growing need for compassionate, end-of-life care in southern New Hampshire. The facility added 6 new private patient suites, increasing its total room count from 10 to 16. As a Registered Nurse in the Community Hospice House, you’ll deliver skilled, compassionate nursing care.  You’ll complete comprehensive assessments, develop individualized care plans, and collaborate closely with physicians, families, and the interdisciplinary team to ensure high‑quality, goal‑directed, cost‑effective care. This role blends clinical expertise, autonomy, and relationship‑centered care to support patients in maintaining comfort and safety.

What You’ll Do

• Complete initial and ongoing assessments, including full physical evaluations and medical history reviews.
• Use the nursing process to plan, deliver, and evaluate skilled nursing care.
• Develop individualized care plans with measurable goals, involving patients and families in all planning and revisions.
• Coordinate interdisciplinary care, communicating significant changes to physicians and obtaining necessary orders.
• Teach and counsel patients and families on self‑care techniques, disease management, and safety.
• Prepare timely, accurate clinical notes and maintain strong communication with the care team.
• Support quality outcomes, patient satisfaction, and appropriate utilization of visits.
• Participate in staff meetings, case conferences, and quality improvement activities.
• Maintain professional growth through continuing education and annual mandatory training.
• Precept students when requested and provide excellent customer service.
• Perform other duties as assigned.

Who You Are

• A Registered Nurse with an Associate Degree in Nursing (BSN preferred).
• Licensed and in good standing with the New Hampshire Board of Nursing.
• Equipped with at least one year of recent acute care experience.
• Certified in BLS/CPR.
• Skilled in patient assessment, care planning, and communication.
• Familiar with EMR systems (Epic preferred).
• Compassionate, adaptable, and committed to delivering high‑quality, patient‑centered care.
