Full time 36 or 40 hours per week. Schedule will either be 7:00pm - 7:00am or 11:00pm - 7:00am including every other weekend rotation & holiday coverage.

What You’ll Do

  • Complete initial and ongoing home assessments, including full physical evaluations and medical history reviews.
  • Use the nursing process to plan, deliver, and evaluate skilled nursing care.
  • Perform accurate OASIS assessments and ensure documentation reflects patient status and care needs.
  • Develop individualized care plans with measurable goals, involving patients and families in all planning and revisions.
  • Coordinate interdisciplinary care, communicating significant changes to physicians and obtaining necessary orders.
  • Teach and counsel patients and families on self‑care techniques, disease management, and safety.
  • Prepare timely, accurate clinical notes and maintain strong communication with the care team.
  • Support quality outcomes, patient satisfaction, and appropriate utilization of visits.
  • Participate in staff meetings, case conferences, and quality improvement activities.
  • Maintain professional growth through continuing education and annual mandatory training.
  • Precept students when requested and provide excellent customer service.
  • Travel within assigned territory, occasionally assisting outside the usual area.
  • Perform other duties as assigned.

Who You Are

  • A Registered Nurse with an Associate Degree in Nursing (BSN preferred).
  • Licensed and in good standing with the New Hampshire Board of Nursing.
  • Equipped with at least one year of recent acute care experience.
  • Certified in BLS/CPR and able to meet driving, insurance, and background requirements.
  • Skilled in patient assessment, care planning, and communication.
  • Comfortable working independently in the home setting while collaborating with an interdisciplinary team.
  • Familiar with OASIS, home health reimbursement, and EMR systems (Allscripts preferred).
  • Compassionate, adaptable, and committed to delivering high‑quality, patient‑centered care.
