RN Community Liaison (Homecare)

Make a meaningful impact by connecting patients, providers, and communities to exceptional care as an RN Community Liaison with our Homecare Team. Novant Health is seeking a relationship-driven professional who is passionate about building trusted partnerships with physicians, healthcare facilities, and community organizations while expanding access to home health services. In this role, you'll serve as a key ambassador, educating referral sources, strengthening strategic relationships, and ensuring patients receive the right care at the right time. • Education: 4 Year / Bachelor's Degree, preferred. Graduate of an accredited school of nursing required.
• Experience: Two years sales and or marketing experience in a health-related field, preferred.
• Licensure/Certification:  Current RN licensure in appropriate state, required. Valid Driver's License in appropriate State, required. 

• Develop and maintain strong relationships with physicians, practice groups, hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, and other referral partners to promote home health services.
• Serve as the primary liaison for referral sources by responding promptly to inquiries, addressing concerns, and delivering exceptional customer service.
• Educate healthcare providers and community partners on home health programs, patient eligibility, referral processes, and available services to increase awareness and appropriate referrals.
• Collaborate with clinical and operational teams to support patient transitions of care, strengthen referral networks, and drive growth while ensuring an outstanding patient and customer experience.
