    Description

    Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.

    This RN will be working with the Heart Failure and Advanced Heart Failure team in addition to working with LVAD and heart transplant patients

    • Development and implementation of IPD critical Pathways
    • IPD discharge planning
    • Facilitating and managing the use of home care
    • Triage of OPD and IPD medical issues with referral to appropriate health care professionals
    • Communications with insurance companies and referring physicians
    • Quality assurance projects
    • Promoting health maintenance education and follow-up
    • Donor & Recipient Family and patient management

    Location: Detroit

    Hours: 630AM-5PM, 4- 10-hour shifts. No weekends or on-call requirements.

    EDUCATION/EXPERIENCE REQUIRED:

    • Bachelor's degree in nursing preferred.
    • Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.
    • Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
    • Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations.
    • For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or OR preferred.

    CERTIFICATIONS/LICENSURES REQUIRED:

    • Current RN license in the State of Michigan.
