Education and Training: 

Must be a Registered Nurse in the state of Michigan. Must possess leadership ability and the willingness to cooperate with other department heads and personnel. Must be in good mental health. 

Duties and Responsibilities:

  1. Receive report from off-going Neighborhood Nurse; give report to CNA’s; and give report to Charge Nurse toward end of every shift.
    1. Reporting of residents’ conditions both orally and in writing, with special emphasis on changes in residents’ condition, accidents, problems, etc.
    2. Confer with Charge Nurse, informing them on any unusual problem and/or any new information.
    3. Explain special needs of the residents. Answer any questions of the CNA’s. Consult with Charge Nurse as necessary.
  2. Report acute changes to charge immediately.
  3. Make rounds, visit residents, obtain information; checking all critical residents first.
  4. Directly supervise staff and report improper behavior.
  5. Set up, administer, and chart medications per facility policies.
  6. Order medications from pharmacy as needed.
  7. Initiate New Orders ASAP.
  8. Ensure antibiotics are initiated and administered the shift they are ordered.
  9. Assist with noting doctor’s orders as needed.
  10. Check the BM list for laxatives.
  11. Pull discontinued medications as soon as they are discontinued.
  12. Monitor and hang IV’s as needed. Hang tube feedings and flush as needed.
  13. Check with team members frequently, assisting with problems and difficulties as the arise.
  14. Administer treatments and chart in ETAR.
  15. Review and make recommendations of treatments to wound nurse.
  16. Supervise the care of critical/very ill residents and assist with care as time permits.
  17. Provide resident care as needed: assist with transfers, answer call lights, feed, toilet, etc.
  18. Monitor snacks/ice water to be passed in a timely manner.
  19. Review POC and make rounds to ensure all assigned tasks are completed: I&O’s, BM’s, vital signs, weights, etc.
  20. Assist Charge Nurse with admissions, discharges and transfers. Assist with discharge summaries as directed.
  21. Review with Charge Nurse any medication administration issues. IE: swallow problems.
  22. Complete resident incident report and notify responsible party.
  23. Ensure charts are completely documented on all incidents.
  24. Make recommendations to management regarding annual evaluations.
  25. Take part in nursing conferences, staff/resident teaching and staff meeting/in-service programs.
  26. Make sure residents are up and ready for appointments.
  27. Assist in developing Resident Care Plan with Interdisciplinary Care Plan Committee, resident and family.
  28. Initiate a Falls Intervention with every fall on your unit.
  29. Ensure that all extra resident belongings are packed, labeled, sealed and sent with the resident or family or given to maintenance so they will not be misplaced.
  30. Requisition supplies and services as needed.
  31. Report for your shift 15 minutes to the hour: 0600, 1400, and 2200.  

 Other duties/responsibilities as directed by the Administrator, Director of Nursing, Charge Nurse, or ADON’s.
