    RN Diabetes Educator

    Full-Time | Days | Diabetes Clinic | University Medical Center

    Are you passionate about helping patients take control of their health and improve their quality of life? As an RN Diabetes Educator, you'll serve as a trusted clinical expert and patient advocate, helping individuals with Type 1 and Type 2 diabetes gain the knowledge, skills, and confidence needed to successfully manage their condition.

    Working closely with endocrinologists, advanced practice providers, pharmacists, dietitians, care coordinators, and other healthcare professionals, you'll provide personalized education, support innovative diabetes technologies, and play a key role in improving patient outcomes. This is more than an education role, it's an opportunity to transform lives through coaching, collaboration, and evidence-based care.

    What You'll Do

    Educate & Empower Patients

    • Provide individualized and group diabetes self-management education.

    • Assess patients' knowledge, health literacy, learning needs, barriers, and readiness to manage their condition.

    • Educate patients on insulin administration, injectable therapies, oral medications, glucose monitoring, nutrition, physical activity, and lifestyle changes.

    • Reinforce treatment plans and help patients establish realistic and achievable self-management goals.

    • Support patients through medication adjustments and treatment changes directed by providers.

    Advance Diabetes Care & Technology

    • Provide education and support for Continuous Glucose Monitoring (CGM) systems, insulin delivery devices, glucose meters, and other diabetes technologies.

    • Help patients confidently adopt and utilize new diabetes management tools.

    • Stay current on emerging diabetes therapies, technologies, and evidence-based practice guidelines.

    Coordinate Comprehensive Care

    • Monitor patient progress through follow-up visits, phone outreach, care coordination, and electronic communication.

    • Identify patients who may benefit from additional education, nutrition services, medication management, or specialty referrals.

    • Collaborate with providers and the interdisciplinary care team to develop individualized care plans that improve patient outcomes.

    Serve as a Clinical Resource

    • Act as a subject matter expert for diabetes management, medications, technology, and patient education.

    • Provide clinical guidance and coaching to healthcare staff regarding diabetes care and self-management strategies.

    • Support the development of educational resources, workflows, and clinic-based programs.

    Improve Outcomes & Drive Innovation

    • Support population health initiatives aimed at improving diabetes outcomes and closing care gaps.

    • Participate in quality improvement projects focused on patient engagement, access to care, and reducing diabetes-related complications.

    • Help implement innovative approaches that enhance diabetes education and patient self-management.

    Qualifications

    Required

    • Bachelor of Science in Nursing (BSN)

    • Active Louisiana Registered Nurse (RN) License

    • Current American Heart Association (AHA) Basic Life Support (BLS) Certification

    • 2-7 years of Registered Nurse experience in a clinical setting

    Preferred

    • Experience providing diabetes education or caring for patients with diabetes

    • Experience with Continuous Glucose Monitoring (CGM), insulin therapy, and diabetes technology

    • Experience in Endocrinology, Ambulatory Care, Chronic Disease Management, Population Health, or Care Coordination

    • Experience with patient education, quality improvement, or healthcare outreach initiatives

    • Master's Degree in Nursing, Public Health, Healthcare Administration, or related field

    • Formal diabetes education training

    Licensure & Certifications

    Required

    • Registered Nurse (RN) License

    • Basic Life Support (BLS) Certification

    Preferred

    • Certified Diabetes Care and Education Specialist (CDCES)

    Candidates who do not currently hold the CDCES credential should demonstrate a willingness to pursue certification with organizational support, training, and professional development opportunities.
