Job Description:
Conduct comprehensive assessments to gather data on patients' physical, emotional, and social health status.
Develop and implement individualized care plans with specific goals and interventions based on assessment data.
Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols.
Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress.
Educate patients and families about health conditions, treatment plans, and self-care strategies to empower them in managing their health.
Facilitate effective hand-off communication by clearly and concisely transferring patient information during shift changes or transitions between departments.
Collaborate with healthcare professionals through regular communication and teamwork to ensure coordinated and optimal patient care.
Maintain accurate and timely documentation of patient interactions, treatments, changes in condition, and care plans.
Engage in continuous professional development by attending workshops, pursuing relevant certifications, and reflecting on practice to maintain clinical competencies and improve patient care.
Oversee daily operations and patient care, ensuring compliance with policies, managing staffing needs, providing leadership, and addressing issues.
Perform point of care testing to provide immediate diagnostic results, enabling timely decision-making and treatment adjustments.
Participate in performance improvement projects by identifying areas for enhancement, implementing changes, and evaluating results to improve patient care quality and outcomes.
