JOB DESCRIPTION About Facility: o 2320 E 93rd St., Chicago, IL o Beds: 205 o Level II NICU o No trauma designation Unit Information: o Beds: 35 in the ED Census? Average patients/day? o Minimum Years of Experience required? 3 years of acute care case management experience o Will you accept a first-time traveler? Yes Patient Types/Common Diagnoses: o This is an underserved population, mostly adults and geriatrics. Will see complex cases like unhoused and low-income patients. Could also see children and adolescents in the ED o It is not uncommon to have patients that utilize the ED as urgent care. o Will be assigned to the ED. Possible to float to inpatient units to help cover if call-outs, but ED will be primary area Case Load: o Around 20, give or take a few Required License, National Certification, Certs: o No specific certifications required. Will accept ASN or higher. Skills required: o Ok if limited or no ED Case Management experience; will be trained in ED once here o Working with multidisciplinary team, navigating various insurance plans, assisting with facility initiatives. o Will be instructed on programs and resources available once here. A big push right now to connect patients that utilize ED as urgent care with resources o Soft skills: Good assessment skills a must, comfortable finding resources for patients, good critical thinking skills, good interpersonal skills. MUST be a team player o Preferred: ED RN experience a plus, but not required Support within the Department: o Leadership o Will be solo ED RN CM, but several inpatient CM here if have questions o CM assistant o Hospitalists 24/7 Technology/Equipment: o EMR: EPIC o Careport for referrals Floating: If yes, where? o Possible to float to inpatient units Orientation (facility and unit): o NEO o 3 shifts of orientation in department; possible for 1-2 additional shift if needed Scheduling: o Weekend rotation: Every 3rd weekend o On call? No o Holiday Expectations: 50% or 2 holidays in contract possible o Approval of RTO: to go through manager o Shift times: 0700-1900 o Schedule cycle: Self-scheduling with manager review and approval.
