An established home health provider is seeking a Field Registered Nurse (RN) to support patient care through follow-up visits and OASIS documentation. This is a great opportunity for an RN who enjoys autonomy in the field while being supported by a structured, point-based productivity model.

The Role
The RN will provide skilled nursing visits in patients' homes, focusing primarily on follow-ups and ongoing care management. This role operates on a point system (approximately 30 points per week) and offers consistent volume, flexibility, and strong support from the clinical team.

Key Responsibilities

• Perform follow-up skilled nursing visits in patient homes
• Complete accurate and timely OASIS documentation
• Monitor patient progress and adjust care plans as needed
• Coordinate with physicians and interdisciplinary team
• Ensure compliance with all state, federal, and agency regulations

Requirements

• Active RN license
• OASIS experience (required)
• Home health experience strongly preferred
• Ability to work independently in a field-based setting
• Strong documentation and time management skills

Compensation & Benefits

• Base salary: $80,000 – $85,000
