Registered Nurse (RN) - Home Health
We are seeking a compassionate and experienced Registered Nurse (RN) to join our Home Health team as a Case Manager. As a Field RN, you will be responsible for providing high-quality nursing care to patients in their homes, directing the activities of Home Health Aides, and ensuring that patients receive comprehensive and coordinated care.
Responsibilities:
• Assess, plan, implement, and evaluate patient care plans in accordance with physician orders and nursing standards
• Conduct home visits to assess patient conditions, provide education, and monitor progress
• Develop and implement individualized care plans to meet the unique needs of each patient
• Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
• Supervise and direct the activities of Home Health Aides
• Maintain accurate and up-to-date patient records
Requirements:
• Current RN licensure in the state of practice
• 1-3 years of experience in home health or a related field
• Strong clinical and communication skills
• Ability to work independently and as part of a team
• Knowledge of home health regulations and standards
