Summary
The RN Wellness Coach reports to the Practice Manager and provides professional nursing care, preventive health coaching, chronic disease management, and wellness program development in support of physician-directed medical care within Beacon Health System Paragon Worksite Clinics.
This role blends direct patient care, health coaching, wellness education, occupational health support, and strategic employer wellness programming to improve individual and population health outcomes. The RN Wellness Coach supports onsite/nearsite employer clinics by delivering high-quality nursing care, one-on-one and group wellness coaching, disease prevention strategies, chronic condition management, and client-specific wellness initiatives.
The RN Wellness Coach works collaboratively with physicians, organizational leaders, employer clients, HR teams, and patients to develop, implement, and evaluate innovative health promotion programs focused on improving employee health, reducing chronic disease burden, increasing workplace wellness engagement, and enhancing overall clinic value.

Clinical Nursing Care & Patient Support
Wellness Coaching & Chronic Disease Management
Coach patients individually and in small groups to achieve health and wellness goals related to:
Weight management
Nutrition
Exercise
Stress management
Smoking cessation
Work/life balance
Chronic disease prevention and management
General lifestyle improvement
Develop personalized wellness plans based on patient goals, health risks, and physician recommendations.
Monitor patient progress and adjust coaching strategies accordingly.
Implement disease management plans for conditions such as:
Diabetes
Hypertension
Obesity
Cardiovascular risk
Tobacco dependence
Provide guideline-based health counseling and evidence-based behavior modification support.
Support long-term patient accountability and sustained lifestyle changes.
Program Development & Population Health
Develop and implement comprehensive wellness programs for employer populations including:
Weight management
Diabetes prevention and education
Tobacco cessation
Stress reduction
Preventive screenings
Healthy lifestyle initiatives
Collaborate with employer clients, HR leaders, and organizational stakeholders to integrate wellness into workplace culture.
Design chronic disease education programs and population-specific health interventions.
Lead wellness classes, seminars, workshops, and off-site educational events.
Analyze biometric, lab, and health risk assessment data to customize wellness initiatives.
Track participation, outcomes, engagement metrics, and program ROI.
Support employer-specific strategic health initiatives that improve workforce wellness and reduce healthcare risks.
Administrative, Scheduling & Documentation Support
Communication & Collaboration
Serve as a wellness and clinical liaison between:
Physicians
Practice leadership
Employer clients
HR departments
Clinical staff
Patients
Communicate health trends, risks, and opportunities to organizational leadership.
Promote patient engagement and employer wellness participation.
Build strong relationships with clients and stakeholders to advance wellness culture.
