We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities.
The RN Case Manager will coordinate and manage care for patients with end-stage illnesses, ensuring comfort-focused care in various home-based settings. This role requires strong clinical judgment, excellent communication skills, and the ability to provide holistic care to patients and their families.
End-stage hospice patients
Care provided in:
Patient homes
Assisted Living Facilities (ALF)
Skilled Nursing Facilities (SNF)
Group homes
Independent living communities
Two hospitals providing General Inpatient (GIP) hospice care
Average Daily Census: ~105 patients
RN Case Managers: 7 (each managing 14 16 patients)
CNA Support: 6 CNAs
Manage a caseload of hospice patients in the community
Provide comprehensive nursing assessments and symptom management
Develop and implement individualized care plans
Coordinate care with physicians, hospice staff, and interdisciplinary teams
Educate patients and families regarding disease progression and end-of-life care
Document patient care accurately in the EMR system
Provide compassionate support to patients and families during end-of-life care
Minimum Experience: 2 years RN experience
Travelers: Experienced travelers preferred
Licensure: Active Registered Nurse (RN) license
Certifications:
BLS (Basic Life Support)

Reliable transportation for community visits
Hospice or end-of-life care experience preferred
Strong patient assessment and care coordination skills
Ability to work independently in a community-based setting
Excellent communication and compassionate patient care
