    Location: Chicago, IL — field-based with regular office time (222 S. Morgan St.)

    Travel: Extensive daily local travel required (~80% field / 20% office)

    Position Summary

    We're looking for an experienced RN Case Manager to join our clinical team. This role is primarily field-based, conducting patient assessments and re-assessments in clients' homes throughout Chicago, while managing an active caseload and serving as a clinical resource for our field nursing staff. The ideal candidate combines strong critical care clinical judgment with the flexibility and independence needed to manage a mobile, client-facing schedule.

    Key Responsibilities

    • Conduct comprehensive new patient (start of care) assessments in clients' homes
    • Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs
    • Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care
    • Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases
    • Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes
    • Ensure care plans and documentation meet regulatory and Joint Commission compliance standards
    • Identify and escalate clinical concerns or changes in condition appropriately
    • Participate in office-based team meetings, case conferences, and administrative work (~1 day/week equivalent)

    Qualifications

    Required:

    • Active, unrestricted RN license in the state of Illinois
    • Critical care clinical experience
    • Reliable personal vehicle, valid driver's license, and current auto insurance
    • Comfortable with extensive daily driving across the greater Chicago area
    • Negative TB Test and Physical
    • Current CPR Certification
    • Strong assessment, critical thinking, and clinical decision-making skills
    • Excellent communication skills — comfortable being a clinical resource for peers
    • Proficiency with electronic documentation systems

    Preferred:

    • Minimum 1 year of ICU experience
    • Home care, hospice, or case management experience
