Job Title: RN - Home Care

Job Summary:
Provides skilled nursing care to patients in their homes based on individualized care plans and clinical needs. Assesses patient conditions, administers treatments, manages ongoing health needs, and coordinates care with physicians, patients, and families.

Education & Experience:

• Associate’s or Bachelor’s Degree in Nursing required.

• 1–2 years of home health, home care, medical-surgical, or community nursing experience preferred.

• Experience providing independent patient care in a home setting is preferred.

Licenses & Certifications:

• Active Registered Nurse (RN) license required.

• BLS certification required.

• Additional home health or specialty certifications preferred based on patient needs.

Key Responsibilities:

• Perform comprehensive assessments of patients in the home and identify changes in condition.

• Develop, implement, and evaluate individualized nursing care plans.

• Administer medications, wound care, IV therapy, and other prescribed treatments.

• Monitor patient progress and identify potential complications requiring medical intervention.

• Educate patients and families on medications, treatments, disease management, and safety.

• Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.

• Maintain accurate clinical documentation and follow infection control, patient safety, and home care standards.
