Coverage Area: Up to 50 miles from Moline homebase

Pay: $50/hour
Schedule: Full-time, 40 hours per week

Overview

We are hiring an RN Home Health in Moline, IL . This is a full-time opportunity for an experienced Registered Nurse with a strong home health background who is comfortable working independently in the field while coordinating closely with patients, families, and interdisciplinary care teams.

This role is ideal for an RN with hands-on experience in Homecare Homebase , OASIS documentation , and home health case management. The position includes local travel and possible on-call responsibilities based on branch and staffing needs.

Key Responsibilities

• Administer skilled nursing care to patients in their homes
• Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs
• Develop, implement, document, and evaluate individualized plans of care
• Coordinate care with patients, families, physicians, referring agencies, and interdisciplinary team members
• Maintain timely, accurate, and detailed clinical documentation
• Educate patients and families on care plans, medications, home safety, diet, and self-care practices
• Ensure continuity and quality of care while maintaining compliance with payer and regulatory requirements

Required Skills & Qualifications

• Active Illinois RN license
• Minimum 2 years of home health experience
• Minimum 2 years of OASIS experience & Homecare Homebase experience
• Current CPR certification
• Valid driver’s license, reliable transportation & active insurance
• Ability to maintain a flexible schedule and travel locally

Preferred Experience

• Bachelor of Science in Nursing (BSN)
• CHPN certification
• Additional hospice experience
• Strong background in field case management and interdisciplinary coordination
