Position Overview

The RN Home Health Case Manager is responsible for coordinating and providing skilled nursing care to patients in their homes. This role requires strong clinical judgment, exceptional communication skills, and the ability to manage a patient caseload independently while collaborating with an interdisciplinary healthcare team.

Key Responsibilities

  • Conduct comprehensive patient assessments in the home setting
  • Develop, implement, and update individualized patient care plans
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals
  • Provide skilled nursing services including wound care, medication management, disease education, and chronic condition monitoring
  • Educate patients and family members on diagnoses, treatments, medications, and self-care techniques
  • Monitor patient progress and adjust care plans as needed
  • Complete accurate and timely electronic documentation
  • Ensure compliance with Medicare, state, and agency regulations
  • Participate in case conferences and interdisciplinary team meetings
  • Advocate for patients to ensure the highest quality of home healthcare services

Qualifications

  • Current Registered Nurse (RN) license in the State of Utah or eligible for licensure
  • Graduate of an accredited nursing program
  • Minimum of one year of RN clinical experience required
  • Previous home health, hospice, case management, or community health experience preferred
  • Strong assessment, critical thinking, and clinical decision-making skills
  • Excellent communication and interpersonal abilities
  • Comfortable working independently and managing daily schedules
  • Experience with electronic medical records (EMR) preferred
  • Valid driver's license with reliable transportation
  • Current auto insurance meeting state requirements

Education & Certification Requirements

  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program
  • Active Utah Registered Nurse (RN) license
  • Current BLS (Basic Life Support) certification through the American Heart Association or American Red Cross
  • Valid driver's license and acceptable driving record
