RN- Home Health Case Manager

Pride Health is hiring a RN - Home Health to support our client's healthcare facility in Mansfield, Ohio.

Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM |

On-Call/Weekend: 1 week and weekend on-call monthly

Location: Mansfield, OH; Coverage areas: Richland County and surrounding counties

Mileage reimbursed: Yes

Job Type: Contract

Contract Length: 14 Week/s

Pay Range: $52-$54/Hour

• Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Manage and coordinate patient care as a Home Health RN Case Manager.
• Assess patient needs and develop individualized plans of care.
• Coordinate services and communicate with physicians and interdisciplinary healthcare teams.
• Complete accurate and timely patient documentation and follow-up.

Requirements:

• License(s): Current Ohio RN license or Compact RN license
• Certification(s): BLS
• Years of experience:1+
• Skills/Specialties/Technology: HCHB documentation experience preferred
