Position Overview

We are seeking an experienced Registered Nurse Case Manager to provide high‑quality home health services to patients across the Trumbull and Stamford areas. This role focuses on case management, skilled nursing care, and comprehensive patient assessments while ensuring compliance with CMS, state, and agency guidelines.

Schedule & Work Details

  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 40 hours
  • Productivity Expectation: 30 points per week
  • Coverage Area: Trumbull and Stamford, with visit days clustered for efficiency
  • On‑Call/Weekend Requirement: None
  • Mileage Reimbursement: From second patient through last patient
  • Pay Rate: $51 to $53 per hour

Key Responsibilities

  • Manage assigned patient caseloads and provide comprehensive case management services
  • Complete home health admissions, recertifications, and RN evaluations
  • Deliver skilled nursing care including wound care and ongoing patient assessments
  • Develop, implement, and update individualized plans of care in collaboration with physicians and interdisciplinary teams
  • Ensure accurate, timely documentation using Homecare HomeBase (HCHB)
  • Maintain compliance with OASIS requirements, CMS regulations, and agency policies

Required Qualifications

  • Current Registered Nurse license in applicable state
  • Prior home health experience
  • Proficiency with Homecare HomeBase (HCHB) and OASIS documentation
  • Strong clinical assessment and care coordination skills

Preferred Qualifications

  • Previous wound care experience
  • Strong organizational and time management skills
  • Ability to work independently while maintaining collaboration with care teams
