Registered Nurse (RN) – Home Health Job Summary: We are seeking a Registered Nurse (RN) – Home Health to provide skilled, patient-centred care to patients in their homes throughout Mesa County, CO. This position offers a flexible daytime schedule, manageable caseloads, and meaningful one-on-one patient care.

Contract: 13 Weeks
Shift: Monday–Friday, Day Shift + On-Call Rotation
Guaranteed Hours: 40 Hours/Week
Pay Details:
Weekly Pay: $2,306
W2 Pay: $26.50/hour
Weekly Tax-Free Stipend: $1,246
OT Rate: $70/hour
Blended Rate: $57.65/hour
Responsibilities
Perform comprehensive patient assessments.
Develop and update individualized plans of care.
Provide skilled nursing care and appropriate interventions.
Monitor patient progress and report changes to the care team.
Educate patients and families about medications, treatments, and care plans.
Coordinate care with LPNs, therapists, caregivers, and healthcare providers.
Maintain accurate and timely documentation in the EMR.
Participate in an on-call rotation.
Requirements
Active Colorado RN license or active NLC Compact RN license.
BLS/BCLS certification.
Valid driver's license.
Reliable transportation and auto insurance.
Strong assessment and critical-thinking skills.
Ability to work independently and manage a structured caseload.
Strong communication and documentation skills.
Comfortable using EMR systems; HCHB experience preferred.
Previous home health experience is preferred but not required.
