Position Overview

Seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Hot Springs area. This role is responsible for comprehensive patient assessments, care planning, OASIS documentation, coordination of services, and ensuring exceptional patient outcomes. The ideal candidate will have strong home health experience, excellent clinical judgment, and a commitment to delivering high-quality patient-centered care.

Schedule & Work Details

  • Location: Hot Springs, AR
  • Schedule: Monday through Friday, 8:00 AM to 4:40 PM
  • Weekly Hours: 40 hours per week (32-hour minimum guarantee)
  • Pay Rate: $51.00 - $53.00 per hour
  • On-Call Requirement: One weekend every 1-2 months based on agency needs
  • Holiday Coverage: Potential availability on observed holidays as needed
  • Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

Clinical Competence

  • Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing care in accordance with physician orders, agency policies, and state and federal regulations.
  • Supervise and support home health aides and licensed practical/vocational nurses, including assigning tasks and conducting required supervisory visits.
  • Perform comprehensive nursing assessments and identify patient care needs.

Documentation & Care Delivery

  • Complete comprehensive assessments, including OASIS documentation and medication reconciliation, accurately and timely.
  • Deliver high-quality clinical services while adhering to infection prevention and control standards.
  • Maintain accurate, thorough, and timely patient documentation in accordance with agency, payer, and regulatory requirements.
  • Ensure all visit documentation is completed and synchronized according to company policy.

Quality & Care Coordination

  • Conduct initial and comprehensive evaluations to determine home health eligibility and medical necessity.
  • Develop and update patient care plans in collaboration with physicians and interdisciplinary team members.
  • Communicate effectively with agency staff regarding patient needs, visit schedules, care coordination activities, OASIS submissions, and coding requests.
  • Collaborate with physicians, patients, families, and healthcare team members to support quality outcomes and service excellence.
  • Promote patient safety and work to reduce preventable hospitalizations.

Professionalism & Teamwork

  • Maintain a positive, patient-focused attitude and commitment to exceptional care.
  • Attend and actively participate in meetings, case conferences, training sessions, and ongoing education.
  • Assist with orientation and training of new staff members and serve as a preceptor when needed.
  • Participate in performance improvement initiatives, survey readiness activities, and agency quality programs.
  • Demonstrate flexibility with scheduling to meet patient care needs, including participation in on-call and weekend rotations.
  • Follow agency utilization management processes and complete all assigned duties in a timely, professional manner.

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Arkansas.
  • Minimum of one year of recent Home Health nursing experience.
  • Experience completing OASIS assessments and documentation.
  • Current BLS/CPR certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Strong clinical assessment, critical thinking, and care coordination skills.
  • Ability to work independently while managing a field-based caseload.
  • Excellent communication, documentation, and organizational skills.

Preferred Qualifications

  • Previous experience with LHC Group systems and processes.
  • Strong knowledge of Medicare home health regulations and compliance standards.
  • Experience mentoring staff, serving as a preceptor, or supervising clinical personnel.
  • Proficiency with electronic medical record (EMR) systems.
  • Proven ability to build strong relationships with patients, families, physicians, and interdisciplinary team members.
