Job Description – Home Health RN
We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The Home Health RN will assess patient needs, develop individualized care plans, administer medications and treatments, and coordinate with physicians and other healthcare professionals to ensure quality care.
Type: Home Health
Schedule: 8 AM- 4:30 PM
Key Responsibilities:
Perform in-home patient assessments and monitor health status
Administer medications, wound care, and other treatments
Educate patients and families on care plans and disease management
Collaborate with the healthcare team to ensure continuity of care
Document all visits and services in accordance with agency and regulatory guidelines
Other duties as assigned.
Qualifications:
Active Registered Nurse (RN) license.
Minimum of 1 year of clinical nursing experience (home health preferred)
Strong communication and organizational skills
Reliable transportation and a valid driver’s license
Type: full-time contract employment 13-14 weeks with possibility to extend
