Registered Nurse (RN) – Home Health

Duration: 13 Weeks
Shift: Day | 8:00 AM
Schedule: Monday–Friday | 40 Hours/Week | Occasional Weekends & Holidays

Pay Rate:
Traveler: $65–$75/hr

Required Qualifications:

• Current Massachusetts RN license and required credentials — Required
• Current CPR certification — Required
• Minimum 1 year of nursing experience OR 6 months with a BSN
• Successful completion of VNA pre-hire medication test
• Strong computer and clinical documentation skills
• Ability to provide independent home health nursing care
• Home health and IV experience — Preferred
• BSN — Preferred

Responsibilities:

• Perform comprehensive nursing assessments during patient admission
• Assess physiological, psychosocial, environmental, and health-history needs
• Develop, implement, and revise individualized plans of care
• Establish visit frequency and coordinate interdisciplinary services
• Provide skilled nursing care and patient/caregiver education
• Monitor patient progress and report significant changes to physicians and care providers
• Participate in interdisciplinary case conferences
• Supervise the Home Health Aide plan of care every 14 days
• Obtain and clarify physician orders for patient care
• Complete admission, ongoing, and discharge documentation accurately
• Maintain clear documentation of teaching, direct care, and patient response
• Coordinate patient discharge planning with the patient, physician, and care team
• Refer patients to appropriate community resources
• Maintain required HHRG values and support quality outcomes
• Follow clinical, safety, documentation, and compliance standards
