Certified Wound Care Nurse - Direct Hire
Compensation: $96, 000-$120, 000, based on experience
Job SummaryWe are seeking a Certified Wound Care Nurse to assess, diagnose, plan, implement, and evaluate patient care for individuals with wound, ostomy, continence, and heart failure-related needs. In this role, you will coordinate care across interdisciplinary teams, partner closely with patients and families, and apply evidence-based practice to improve outcomes. The ideal candidate brings strong clinical judgment, excellent assessment skills, and a commitment to compassionate, ethical, patient-centered care.
Key Responsibilities
  • Perform gastrointestinal, genitourinary, integumentary, and cardiac assessments to develop, implement, and evaluate individualized plans of care.
  • Administer, monitor, and document therapeutic interventions related to wound, ostomy, continence, and heart failure care.
  • Identify and respond promptly to changes in patient condition to help prevent hospitalizations.
  • Educate patients, families, and caregivers on disease processes, safety, wellness, symptom monitoring, diet restrictions, and medication management.
  • Provide counseling on when to report changes in symptoms to a nurse or provider.
  • Participate in clinical decision-making and collaborate with physicians and multidisciplinary care teams to support the highest quality outcomes.
  • Delegate and monitor care delivered by other members of the nursing and healthcare team.
  • Maintain patient safety standards and demonstrate standard precautions in all aspects of care.
  • Contribute to orientation, education, and professional development of the healthcare team.
  • Participate in performance improvement initiatives and support evidence-based practice recommendations.
  • Maintain departmental and professional competencies, including specialty knowledge and compliance requirements.

Qualifications
Required Experience: Minimum of 2-3 years of nursing experience. Home health experience preferred for WOCN-focused candidates. For heart failure care, 2-3 years of acute care experience in cardiac care, telemetry, or ICU is preferred.Required Education: Graduate of an accredited nursing program. Bachelor’s degree in nursing preferred. Wound specialty education from an accredited program (such as WOCN or WCC) is preferred.Licensure and Certifications: Current State of New Jersey RN license required. Current CPR and BLS certifications required. Valid driver’s license and proof of auto insurance required. Board certification in wound care (such as CWCN, WCC, CWS, or CWCA) is preferred within 1 year of hire. Heart failure certification from an accredited program is preferred.Core Competencies
