Position Overview
Seeking a dedicated and experienced Registered Nurse (RN) to provide high-quality home health care services throughout the greater Needham service area. The RN will be responsible for comprehensive patient assessments, care planning, clinical oversight, and coordination of services to ensure patients receive exceptional care in their homes. This role requires strong clinical judgment, independent decision-making, and excellent documentation skills.
Schedule & Work Details
- Location: Coverage area includes Arlington, Belmont, Lexington, Lincoln, Waltham, Watertown, Wayland, Weston, and Winchester, MA
- Schedule: Monday through Friday, 8:00 AM - 4:30 PM
- Hours Guaranteed: 32 hours per week
- On-Call Requirement: Yes
- Weekend Requirement: Rotating weekends, approximately every 5 weeks
- Mileage Reimbursement: Yes, between patient visits
- Pay Rate: $51.00 - $53.00 per hour
- Dress Code: Business casual (no jeans); scrub color is not specified
Key Responsibilities
- Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and other healthcare professionals.
- Conduct comprehensive patient assessments, including OASIS assessments and medication reconciliation.
- Deliver skilled nursing services in accordance with physician orders, agency policies, infection control standards, and regulatory requirements.
- Coordinate care with interdisciplinary team members to ensure optimal patient outcomes.
- Supervise and provide direction to Home Health Aides and Licensed Practical/Vocational Nurses in accordance with state and federal regulations.
- Perform required supervisory visits and provide ongoing clinical guidance to support staff.
- Monitor patient progress, identify changes in condition, and communicate updates to the care team as appropriate.
- Complete accurate, detailed, and timely clinical documentation in compliance with agency and payer requirements.
- Ensure all visit documentation is synchronized and submitted according to agency policy.
- Educate patients and caregivers regarding disease management, treatment plans, medications, and safety measures.
Required Qualifications
- Active and unrestricted Massachusetts Registered Nurse (RN) License.
- Current ability to practice within the scope of Massachusetts RN licensure.
- Experience performing skilled nursing assessments and care planning.
- Strong clinical documentation and organizational skills.
- Ability to work independently and effectively manage a home-based patient caseload.
- Reliable transportation and ability to travel throughout the assigned coverage area.
Preferred Qualifications
- Home health experience.
- Experience completing OASIS assessments and documentation.
- Experience caring for post-acute patients in skilled nursing facilities, nursing homes, assisted living settings, or home health environments.
- Ability to work autonomously with minimal supervision.
- Quick learner with the ability to onboard and become productive rapidly.
- Strong communication and care coordination skills.
