OneTeam Healthcare is seeking a compassionate and dependable Registered Nurse (RN) to provide care to residents within local retirement and senior living communities. This role offers the flexibility and autonomy of home health while minimizing travel and allowing more time focused on quality patient care.

Responsibilities

• Provide skilled nursing care and comprehensive patient assessments
• Develop, implement, and update individualized care plans
• Administer medications, treatments, and wound care as ordered
• Monitor patient condition and communicate changes to physicians and care team
• Educate patients and families regarding diagnoses, medications, and care plans
• Accurately document visits and care provided within EMR system
• Collaborate with interdisciplinary team to ensure quality patient outcomes

Qualifications

• Active RN license in good standing
• Previous home health, assisted living, SNF, or senior care experience preferred
• Strong assessment and clinical skills
• Excellent communication and organizational abilities
• Compassionate, dependable, and patient-focused mindset
• Ability to work independently while collaborating with a clinical team
• Current driver's license and reliable transportation
