Job Summary: 

Responsible for assessing, diagnosing, planning, implementing, and evaluating patient care needs with selected disorders through outcome identification and evidence-based practice.  Coordinates patient care and communicates and collaborates extensively with healthcare teams, patients and/or patient’s family or significant other(s).  Maintains and demonstrates high standards of professional ethics.

·     Performs a gastrointestinal, genitourinary, integumentary or cardiac assessment of the patient, leading to development, implementation and evaluation of a plan of care 

·     Administers, monitors and documents therapeutic interventions and regimes related to wound, ostomy, continence or heart failure.

·     Effectively manages rapidly changing situations.

·     Participate in clinical decision-making.

·     Promptly identify and address relevant changes in clinical status to prevent hospitalizations

·     Educates patient and family/responsible party regarding disease process, individual care needs, wellness, safety issues, etc.

·     Provide education and counseling to patients and/or caregivers

      Diet restrictions, Medication review, Heart Failure symptoms

      When to report change in symptoms to nurse/ provider

·     Delegates and monitors care rendered by other members of nursing department and healthcare team.

·     Demonstrates standard precautions and patient safety principles in practice.

Position Qualifications Required:

Required Experience:

WOCN: Minimum of 2-3 years of nursing experience. Home Health experience preferred.




Heart Failure: Minimum of 2-3 years of acute care nursing experience in cardiac care, telemetry or ICU 




Excellent assessment skills and clinical judgement.

Required Education:

Graduate of an accredited nursing program.

Baccalaureate Degree preferred.  

Wound Specialty Education preferred from accredited program (WOCN, WCC)

Training / Certification / Licensure:

Current State of New Jersey nursing license to practice as a registered nurse.  Current CPR certification and BLS.

Must have current proof of valid driver's license and auto insurance. Valid New Jersey RN license Approved licenses must be presented and maintained in accordance with the State of New Jersey Licensing Board. Specialty Certification, CPR certified 




WOCN: Board Certification in Wound Care (CWCN, WCC, CWS, CWCA) desired within 1 year of hire. Refer to the Educational/Regulatory Requirement (Department Specific) policy in the Nursing Administrative Manual.




Heart Failure: Certification in Heart Failure from an accredited program preferred
