RN Admissions Nurse - Polk County, FL
Compassionate Care Hospice (CCH), an Amedisys Company
**This position is based in Lakeland, FL and requires daily travel to assigned area(s). Schedule: Monday-Thursday, 12:00 PM - 11:00 PM.

Salary Range:
$82,000–$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Responsibilities:
Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocialand spiritual needs.
Coordinates the establishment of the initial plan of care with the attending physician, medical directorand the interdisciplinary team.
Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
Functions as a liaison between disciplines and facilities during the admission process.
Develops, preparesand maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Performs other duties as assigned.
Qualifications:
Required:
Hospice or home health experience
Current RN license in the state(s) where you will work
Minimum 1 year of clinical RN experience in an acute healthcare setting
Current CPR certification
Preferred:
Strong communication and interpersonal skills
Ability to work independently and manage multiple priorities
Comfortable with daily travel between facilities
Proficient in electronic medical record (EMR) systems
